Respiratory Outcomes Management) disease management program, aimed at improving patients’ quality

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

Consults with patient’s physician regarding patient’s health and wellbeing when additional
therapies or interventions may be needed. * Consults with physician in the event of adverse
reaction to therapies introduced. * Monitors patient’s physiological responses to therapy and
testing procedures through patient vital signs and physiological signs and symptoms. *
Participates, on an ongoing basis, in traditional as well as virtual learning and educational
programs. * Demonstrates knowledge and confidence in subject matter related to PPHM CROM
program (as well as additional educational programs offered to patients – at the discretion of the
Respiratory Therapists manager) to trainees and other health care personnel. * Educates patients
on each respiratory topic of education as outlined in PPHM CROM program visit schedule.o

Maintains an understanding of the subject matter as set forth during orientation and field training.
* Performs assessments and educates patients in the patient’s home.
